I give permission for my child to work with electrical equipment (including lighting equipment).
I give permission for my child to work on-location under supervision of staff.
I give permission for my child’s image and voice to appear in future promotional or PR material, which may include video.
I give permission for my child to use the internal Birmingham City University transport service to travel to locations whilst on the Future Creatives programme.
I give permission for my child to take part in Birmingham City University’s Future Creatives programme.
I have read the terms and conditions of booking and the Code of Conduct and understand that if my child contravenes this agreement, and is asked to leave, or is otherwise obliged to transfer or leave Summer School early, I will need to make arrangements for my child to be collected without delay or meet any costs incurred in making alternative travel arrangements.
